I had planned to put the Plan ID as the "Client Number". I've never seen any other number that it could mean. I assumed that's what they mean.

You're going to fill out the transfer form at both Ascensus and Fidelity? Will that create confusion. I know in the past whenever people tried to do the transfer process on Fidelity or Schwab's end, once they reached out to Ascensus, Ascensus wouldn't play along with them. So far I haven't heard of anyone having a problem getting Ascensus to send the checks using their own transfer process, either the old or new one. That was never the problem with Ascensus. The problem was them insisting on you terminating the employee and then promising to issue a 1099 for the transaction.
I am filling out the Transfer of Assets form Fidelity requires and the new Ascnesus form mentioned above. When I called Fidelty they told me I could sent them both to them, and there is at least one person recently upthread who states that Fidelity was able to do the transfer for them since the new Ascensus form has come out; that said, I have called Fidelity numerous times since July and the Fidelity rep I spoke with seemed the last knowledgable out of all of the Fidelity reps I have spoken to, so if anyone who has successfully recently made this transfer has info on what works best, please let us know!

The Fidelty rep also told me to send the forms to the mailing address on the transfer of assets form. I asked about sending to the overnight address and he told me that since that is not on the form, he could not guarantee I can use that address. I think some here have used that address to get the transfer of assets form to Fidelity and it was fine?

Also, can anyone who confirm on the Transfer of Assets form to Fidelity, for Account Owner/Entity name do I put my name, my business name, or both? I did call Fidelity, but did not get a real answer.
Are you transferring to a Fidelity prototype (Self-Employed 401k) or non-prototype (third-party administrator) plan? If it's a non-prototype, contact their non-prototype department. They are very helpful. (800)544-5373 M-F 8a-8:30p EDT.

I only used the transfer of assets form from Fidelity when I left Vanguard. I did not require any paperwork to Vanguard. I took my paperwork (including account opening information) to a Fidelity office who signed as a medallion signature guarantee. Took a few days for the non-Roth and 2 weeks for the Roth (Vanguard said they didn't receive it initially).
