medicare will pay 100% of the first 20 days and most medicare supplements will pay the copayment between day 21-100.No reason to pay anything out of pocket because i believe every SNF takes medicare.The case worker at the hospital will know which are the nicest SNF in the area but if you don't intervene they will usually just choose facility based on bed availability at SNF and location.There are definitely SNF that are nicer than others but with original medicare and a supplement the cost will be same no matter which one they put him in.
Thanks... yes a case worker reached out to me just today, initially they wanted him to recover at home, but he is in no condition as I believe he still needs 24/7 care as he has zero mobility right now.
With an elderly frail relative (all situations are different, granted) I found the time spent in a good rehab facility to be a huge help in getting that person functional again. I wouldn't allow the hospital to release a patient with no mobility to home (you can challenge a discharge under Medicare rules).

If possible, do as much research as you can to figure out which facility has the best rehab options. Some places will absolutely be better than others.

In my experience, hospital case workers were far less useful than you'd hope in picking the "best" facility. I had personally spent a lot more time in the area care facilities than any case worker I talked with .... the doctors might have a better idea of which SNF is better, since they might send a patient to a facility, and then see them again after rehab. What the case workers are good at is telling you what facilities actually have beds, and providing contact information.
