There are two ways to use it. You can use it from the amex app itself and send people money directly. I do not use this feature to avoid sharing my contacts with Amex.
The other way is two steps. First step is to load money onto your Venmo account balance from the amex spend account. Then from there you can use it as "normal" venmo money. I do this even though it is an extra step.
